Participant Eligibility

Eligible participants will receive a prescription card to buy fruits and vegetables at Stop N Shop in Leland, MS. The purpose of this program is to improve dietary health through increased consumption of fruits and vegetables, reduce individual and household food insecurity, and reduce healthcare use and associated costs.

Population Served

Population Served (number of people served, if applicable): Leland Medical Clinic patients that have a diet-related chronic condition (i.e. diabetes, high blood pressure, high cholesterol). They must also have Medicaid or Medicare as their primary insurance or must currently be on SNAP/EBT.
