Update on LMC Behavioral Health Services

Dear Leland Medical Clinic Patient:



Beginning on January 25, 2024, Cay Ely, our Mental Health Nurse Practitioner, will be relocating to a new office and will no longer be providing services at Delta Health Alliance’s Leland Medical Clinic. 

You or your family member’s care and treatment are the most important things to us. We value you as a patient and want to make your continuing medical care as convenient as possible. If you are under treatment or have an upcoming appointment with Ms. Ely, unless notified your care will transition to our other Mental Health Nurse Practitioner Qwana Powell for your next appointments.  As long as you want to continue receiving treatment at the Leland Medical Clinic, your medical records will be maintained by the Clinic in accordance with the Clinic’s normal record-keeping procedures so that they are available for your future visits there. 

Should you wish to have your records transferred to Ms. Ely at her new location(s), please contact her office at (662) 588-3911.

We want to make this transition as smooth as possible for you. Delta Health Alliance and Ms. Ely take our responsibility to patients seriously, and your healthcare needs and well-being are, and will remain, our top priority. We understand you may have questions concerning your care in light of this letter, and we encourage you to call the office at 662- 686-4121 so that we can discuss your questions and needs with you.
